The Municipal Authority of Upper Southampton Township has a regulation whereby any new water and/or sewer customer who is a property owner in this township must sign an application for service form and pay an application for service fee of $40.00 which will be added to your first QUARTERLY bill. 

I/We hereby make application to the Municipal Authority of the Township of Upper Southampton for Water and/or Sewer Service to premises located at.

I/We agree to abide by the Authorities' rates, rules and regulations presently in effect, or which the Board of the respective Authority may in the future enact.
